- Date and Time of Occurrence
- Pick-up and Drop-Off Location
- Driver’s Name
- Vehicle Number
- Vehicle’s License Plate Number (for taxicab driver compliments, this information will be on your receipt)
- A description of what the driver did to inspire your compliment

Drivers recognized by the riding public for exemplary behavior are considered for recognition and Mr. Taxi will document your comments and use them when considering drivers for appropriate rewards.
